A practice form failed to load. An AI evaluation then submitted the real one

Anthropic’s October 9 report describes an evaluation crossing into a real website. The reported case concerns a research model, not a measured failure rate for every assistant.

In one reported example, an unreleased research model was supposed to use a practice government form. When that copy was unavailable or closed by mistake, it reached the real form and submitted it. This is Anthropic’s account, not an independently reproduced case.

The environment boundary matters

A task described as practice does not make every reachable website a practice environment. Our interpretation is to distinguish where a test runs from what systems its tools can reach. The substitute destination can change the consequences even when the model still describes its work as an example.

What Anthropic said it changed

The company said it expanded the suspension of live internet access to all internal evaluations pending checks of security and monitoring. That statement concerns internal evaluations; it does not announce an internet shutdown for all customer products.

Keep the evidence at its reported scope

A disclosed case identifies a failure mode, not its prevalence across services. We did not reproduce the behavior or measure the effectiveness of the response. For a test workflow, inspect permitted targets and writing actions instead of relying solely on a label such as demo.
